Tembusu
Health Centre
To reach out to more needy persons, 4S
relocated its free clinic to Blk 3 Eunos Crescent in mid 2009. The Sathya
Sai Baba (General and Specialist) Clinic, previously functioning at 133
Moulmein Road, ceased operations in June 2009, and the newly set up
Tembusu Health Centre (THC) will undertake the provision of free medical
consultation and treatment to the needy. Operations will commence in mid
September 2009.

THC
occupies 3 units of shophouse at Eunos Crescent. Besides setting up a
clinic at its new premises to deliver free medical care to the needy, THC
will also incorporate a social service component to assist residents with
service needs. This integrated service approach is based on the premise
that "besides medical and health problems that need to be treated,
residents would also need supportive help to alleviate their social and
emotional difficulties".

THC
will offer primary healthcare services such as General Practitioner
service, chronic disease case management and community health promotion.
Support services such as loan of supportive appliances as well as social
work services such as counseling, case management and information and
referral service will also be incorporated.

Community
Health Screening

In year 2001, 4S launched a series of free
health screening sessions in partnership with the Health Promotion Board
and the grassroots organisations in Hong Kah North Division. Free health
screening has since been part of 4S’ community health promotion effort.

The
services of the health check comprise body fat analysis, body mass index,
blood cholesterol test, blood pressure check, blood sugar test and
urinalysis.
Tembusu Rehab Centre

Tembusu Rehab Centre, previously located in
the Bukit Batok polyclinic, was relocated to Blk 715 Jurong West St 71 in
mid 2009. The Centre, under the purview of MOH, provides outpatient
rehabilitation in the form of physiotherapy and/or occupational therapy
for those with physical disabilities resulting from stroke, fracture, head
injuries or other conditions.
Managed by 4S since 2002, the objective of
the Centre is to help the service users regain their functional
capabilities. The Centre is committed to providing the very best and
affordable rehabilitative therapy in a safe, clean and caring environment.
A professional team of Physiotherapist and Occupational Therapist assesses
the client and a treatment plan is developed based on the client’s
individual needs.

Tembusu
Transport
Tembusu Transport, launched in April 2007 and operated
from THC, is an affordable, door-to-door, shared ride, non-emergency
transport
service for residents in the South East District. The target
residents
are those who have difficulties
using
public
transport
because
of
their frailty
and/or
physical
disability
to attend medical
appointments.

This
heavily subsidised transport service offers concessionary fares and is
affordable to the lower income group. It is a joint project of 4S and the
Kampong Ubi-Kembangan Citizens’ Consultative Committee, supported by the
South East Community Development Council and Lorong Koo Chye Sheng Hong
Temple. |
